Valid Passport with a minimum validity of 6 months. This passport must not have any corrections regarding personal identification data. If your passport contains corrections to personal data, you must replace it with a new passport containing the correct information. Corrections on pages 4 or 5 are NOT permitted.

Two (2) recent passport photos, size 3.5 x 4.5 cm, with a white background, of good quality, and with the face clearly proportional at 70% when zoomed in. Both ears must be visible in the photo. Do not wear glasses or soft lenses.

Sponsorship Letter in English from your current employer, which must also state your joining date and start date of employment, along with your salary slips for the last 3 months or a statement of your monthly income.

Attach a copy of the company’s SIUP/NIB (Business License/ID Number) and the Ministry of Law and Human Rights (Kemenkumham) approval if the applicant holds the position of General Manager, Director, President Director, or Commissioner, AND if the travel expenses are covered by the company (usually for business purposes).

Photocopy of ID Card (KTP), Family Card (KK), Marriage Certificate, and Birth Certificate/Name Change Certificate (if any).

School/University Letter of Enrollment (in English) for children who are still attending school/college.

Original Bank Reference Letter in English.

Travel Insurance with a policy value of at least USD 50,000 or EUR 30,000, covering the entire period of the ticketed journey (Coverage Area: WORLDWIDE).

Financial records for the last 3 months (Bank Statement with official bank legalization/stamp). This must be updated just before the appointment date.

Ticket Booking and any internal tickets for travel between countries.

Hotel Booking and a day-by-day travel itinerary explaining the purpose of each day’s activities.

If a child under 18 is traveling with only one biological parent, the child’s birth certificate must be NOTARIZED, and both biological parents are required to be present at the VFS Italy center.

Travel Insurance (minimum coverage of USD 50,000 or EUR 30,000) with WORLDWIDE coverage is mandatory.

If invited by a company in Italy, attach a copy of the inviter’s ID CARD and the inviting company’s Business License (SIUP/equivalent).
