Fill out Form BI-84.

Original Passport and a photocopy of all passport pages (only pages that have stamps and visas).

Two (2) recent studio photos, size 3.5cm x 4.5cm, with a white background.

Flight Tickets, Hotel Bookings, and a Travel Itinerary.

Bank Reference Letter and a full 3-month Bank Statement, including the current month (original copy with the bank’s original stamp on every page).

Sponsorship Letter from your employer (if employed, on company letterhead with a wet-ink signature), and a sponsorship letter from your husband (for housewives), written in English.

If you are a business owner, attach the SIUP (Trade Business License) and TDP (Company Registration Certificate).

Photocopy of ID Card (KTP), Family Card (KK), Marriage Certificate, Birth Certificate, and Name Change/Indonesian Citizenship Certificate (if any).

Photocopy of Student ID Card (if still enrolled in school/college).

Mandatory Document Translation by a Sworn Translator: The following documents must be translated: KTP (ID Card), KK (Family Card), Marriage Certificate, Birth Certificate, SIUP/NIB/KEMENHUMHAM, and Name Change/Citizenship Certificate (if any). If the documents are already bilingual, they do not need to be translated again.
